The Contra Costa County Community Services Department has a number of programs and services to serve you. Please click the links to the left for a description of each.

Children living in a defined target area and whose family meet the Federal/State income guidelines are eligible for our programs. There is no fee.

You may view and print an enrollment application for admission to one of our programs. This requires the free .

Please call (925) 646-5540 for information and an appointment. Then, bring the following information to your appointment:

  1. Verification of Birth
  • Birth Certificate, or
  • Baptismal Certificate, or
  • Medical card, or
  • Passport
  1. Child's immunization record
  2. Verification of income
  • Check stub
  • Unemployment verification
  • TANF recipients please bring Notice of Action
  1. Medi-cal card (if applicable)
  2. Social security cards of all members in the home
  3. Foster parents or legal guardians must bring proof of guardianship.
