If you plan to elect either Kaiser Senior Advantage or Health Net Seniority Plus plan, please contact our office as you will be required to complete either an election form in addition to the Open Enrollment Change form. Not necessary if you are already enrolled. We will also require a copy of your medicare card indicating you are enrolled in Medicare Part A and Medicare Part B. Contact Melaniece at (925) 335-1705 if you need the appropriate election form.
2013 Benefit Statements, 2014 rates and enrollment forms were mailed to all Retirees/Survivors for review of present benefits to the address on record as of September 20, 2013. If you did not receive information, please contact our office to update your address.
